How to request access to records
- Read the manual and complete PAIA Form 2: Request for Access to Record, included in Appendix 4.
- Provide sufficient details to identify the records you are requesting, your contact details, and the other information and supporting documents required by the form. If you are acting for someone else, include proof of your authority to do so.
- Email your completed request to care@pharmacydirect.co.za, addressed to the Information Officer, with the subject PAIA access request.
